Section A – Identification
To ensure that we have accurate information on vessel owners, we need:
- Name of contact vessel owner
- Fisher Identification Number (FIN)
- Business Number (BN)
- Vessel Registration Number (VRN) and vessel name
- Homeport
- All other vessels under the contact vessel owner
Section B – Individual or single organization
This section will help us identify independent and simple corporate structures.
Is the vessel 100% owned by one:
- Person or
- Wholly owned organization
If Yes, we need:
- Name of (each) person and/or wholly owned organization
- Citizenship of (each) person and/or country of incorporation
- Name of (each) organization's sole owner
- Percentage of the licence held by (each) person and/or organization with 10% ownership or more
- Existing debts that are convertible to equity

Section C – Multi-tiered organization
This section is intended to gather personal information on all beneficial owners of commercial fishing licences, including those employing complex corporate structures.
Beneficial owners are individuals who directly or indirectly own or control, in whole or in part, a corporation or an entity other than a corporation.
In the case of a trust, beneficial owners are the trustees, the known beneficiaries, and the settlors of the trust. If the trust is a widely held trust or a publicly traded trust, beneficial owners are the trustees and all persons who own or control, directly or indirectly, 10% or more of the units of the trust.
Beneficial owners cannot be other corporations, trusts or other entities. They must be the individuals who are the owners or controllers of these corporations, trusts or other entities.
For each beneficial owner, including foreign individuals and organizations, we need:
- Full name of each person and/or organization
- Citizenship of each person and/or country of incorporation
- Ownership percentage of each person and/or organization with 10% ownership or more
- Tier of ownership (indirect owners only)
The above information is also needed if:
- Any foreign individual holds an executive position, such as member of the Board of Directors, within any organization in your corporate structure
- Your organization is a related party, in whole or part, to any foreign interests in the fishing industry
- Any shareholder and/or Board of Director member and/or officer of your organization owns, holds a position with and/or serves as a consultant for any foreign organization within the fishing industry
- Your organization has any indebtedness, liabilities and/or obligations owed to a foreign or domestic organization or individual for the purpose of carrying out your fishing enterprise's activities, and whether any debt is convertible to equity
