ONE LOG SHOULD BE SUBMITTED FOR EACH FISHING TRIP WHETHER OR NOT FISH ARE LANDED. LOGS MUST BE SUBMITTED BEFORE HAlL OUT CONFIRMATION NUMBER TO FISH FOR ANOTHER TRIP IS ISSUED (SEE CONDITION OF LICENCE).
A. GENERAL INFORMATION
PLEASE ENSURE THAT ALL THE INFORMATION ON THE TOP-LEFT-HAND PORTION OF THE LOG DOCUMENT IS FILLED IN. THE HAlL OUT CONFIRMATION NUMBER TO FISH MUST BE OBTAINED FROM A DFO APPROVED DOCKSIDE MONITORING COMPANY BEFORE VOU ARE ALLOWED TO GO FISHING FOR BLUEFIN TUNA. A NEW AUTHORIZATION NUMBER IS REOUIRED FOR EACH FISHING TRIP.
B. HOME MANAGEMENT AREA
CHECK THE MANAGEMENT AREA WHERE VOU ORIGINATED (WHERE YOUR ~UOTA ALLOCATION COMES FROM): GULF NEW BRUNSWICK, GULF NOVA SCOTIA, NEWFOUNDLAND, PRINCE EDWARD ISLAND, OUEBEC, ST MARGARET'S BAY, SCOTIA-FUNDY.
C. HAlL AND LAN DING INFORMATION
FILL IN THE DATE AND TlME SAILED AND LANDED, PORT SAI LED AND LANDED, AND THE WHARF SAI LED AND LANDED. ALL OF THIS INFORMATION IN ADDITION TO TAG NUMBERS AND ACCURATE WEIGHTS OF INOIVIOUAL FISH MUST BE HAl LED TO THE OPERATIONS
CENTRE BEFORE LANDING IN OROER TO OBTAIN THE HAlL IN NUMBER. THIS CONFIRMATION NUMBER MUST BE RECORDED IN THIS SECTION OF THE LOG DOCUMENT
D. TRIP SOMMARY
THE INFORMATION IN THIS SECTION APPLIES TO THE TOTAL TRIP ANO MUST BE COMPLETEO EVEN IF NO FISH ARE CAUGHT IT IS ESSENTIAL THAT ALL OFTHE INFORMATION IS FILLEO IN CLEARLY AND COMPLETELY. THE TRIP NUMBER STARTS WITH #1 FOR THE FIRST TRIP OF THE YEAR AND IS NUMBERED SEOUENTIALLY THEREAFTER FOR ALL FISHING TRIPS (INCLUDING THOSE WHERE NO FISH ARE CAUGHT). THE TOTAL WEIGHT OF THE FISH LANDED (LBS DRESSED). CREW NUMBER INCLUDES VESSEL CAPTAIN AND DECK HANDS. INDICATE APPROXIMATE POSITION OF FISHING (LAT AND LONG) AND THE NAFO UNIT AREA AND STATISTICAL UNIT AREA FOR THE AREA WHERE MOST FISHING EFFORT OCCURS DURING A TRIP, EVEN IF THERE IS NO CATCH (SEE MAP AFTER SAMPLE FORMS FOR UNIT AREAS). TOTAL HOURS FISHED INCLUDES TIME SPENT CHUMMING AS WELL AS THE TIME THAT HOOKS ARE IN THE WATER. INDICATE THE MOST FREOUENTLY USED GEAR TYPE. SIZE AND MAKE OF HOOK, GAUGE OF MONOFILAMENT AND # OF UNES FISHED DURING A TRIP (WHEN NO GEAR IS FISHED ENTER NG FOR GEAR TYPE AND 0 FOR /1 OF L1NES FISHED). IN CASES WHERE BAIT FOR TU NA FISHING IS CAUGHT DURING THE TU NA FISHING TRIP, PLEASE INDICATE THE AMOUNT AND SPECIES CAUGHT FOR BAIT.
E. CATCH INFORMATION
ON THIS PORTION OF THE DOCUMENT, EACH L1NE SHOULD CONTAIN INFORMATION ON ONE FISH. ALL FISH CAUGHT AND ALL FISH CAUGHT AND DISCARDED IN A TRIP SHOULD BE ENTERED. FOR DISCARDED FISH, INDICATE IF THEY ARE RELEASED DEAO OR ALIVE.
THE RE IS SPACE FOR 9 FISH: IF MORE THAN 9 FISH ARE CAUGHT IN A TRIP, A SECOND SHEET SHOULD BE USEO TO COMPLETE THE TRIP. IF A BLUEFIN TUNA IS CAUGHT, INOICATE THE EN FORCEMENT TAG NUMBER IN THE SECOND COLUMN. IF OTHER TUNAS ARE CAUGHT, INDICATE THE SPECIES (I.E . BIGEYE, YELLOWFIN, ALBACORE) IN THE BLUEFIN TUNA TAG NUMBER/OTHER TUNA SPECIES COLUMN. IT IS IMPORTANT THAT ALL OF THE INFORMATION IS FILLED OUT CLEARLY AND COMPLETELY, IF THE FISH IS LANDED ROUND, BOTH THE ROUND AND DRESSED WEIGHTS SHOULD BE FILLED IN. THE FLANK LENGTH MUST BE TAKEN WHEN THE FISH IS WHOLE. IF THE FISH IS DRESSED AT-SEA, THE FLANK LENGTH MUST BE TAKEN BEFORE THE FISH IS DRESSED, PLEASE SEE MEASURING INSTRUCTIONS BELOW THE COMMENTS SECTION SHOULD CONTAIN INFORMATION ON GEAR DETAILS, WEATHER, OTHER SIGHTINGS OF FISH, ETC.
FLANK LENGTH: THE CURVED DISTANCE TO THE NEAREST CENTIMETRE MEASUREO WITH A METAL TAPE, FOLLOWING THE CONTOUR OF THE BODY OF THE FISH ALONG THE CENTER (LATERAL) L1NE FROM THE TIP OF THE UPPER JAW TO THE FORK OF THE TAIL THE TAPE MUST BE HELD IN CONTACT WITH THE BODY ALONG ITS ENTIRE LENGTH, PASS OVER THE PECTORAL FIN AND FOLLOW THE TAIL OUT TO THE FORK. THE FISH SHOULD BE LYING ON A FLAT SURFACE.
DRESSED LENGTH (PECTORAL FIN FORK LENGTH): THE CURVED DISTANCE TO THE NEAREST CENTIMETRE MEASURED WITH A METAL TAPE, FROM THE UPPER ANTERIOR INSERTION OF THE PECTORAL FIN, TRACING THE CONTOUR OF THE BODY TO THE FORK OF THE TAIL THE TAPE MUST TOUCH THE UPPER PORTION OF THE PECTORAL FIN , THIS MEASUREMENT IS USED WHEN THE FISH IS IN DRESSED CONDITION (HEAD OFF, GUTTED), THE FISH SHOULD BE LYING ON A FLAT SURFACE.
F. SIGNATURES
PRINT THE CAPTAIN'S NAME AND THE NAME OF BUYER'S COMPANY. BOTH THE CAPTAIN AND THE BUYER (OR AGENT) MUST SIGN THE FORM.
