Note 1: Only fields marked with an asterisk (*) are mandatory and have to be filled when this form is used to upload the information into the ''Gear Tags Repository System''.
Note 2: All fields on this form are mandatory to be filled if the ''System'' is not used to record and submit the information to DFO.
Note 3: As provided by the Protocol, this information should be submitted to DFO prior to the opening date of the fishery for which these tags are issued.
Instructions: please note the following information:
- No more than one original tag number should be entered in one cell
- No letter should be added to the recorded licence or original tag number
- For any original tag number recorded, all mandatory cells must be filled
- The date format should be entered appropriately: dd/mm/yyyy
- Do not remove or switch any column from the table provided above
- Tags issued to fishers not residing in the same DFO Region should not be recorded in the same tab
- Original and replacement tags cannot be mixed in the same tab, must use appropriate tab
- Do not apply a filter to the spreadsheet
- Do not apply formulas to the spreadsheet as this will generate errors when uploading
